Insurance
At Fairfax Oral and Maxillofacial Surgery our primary goal is to deliver the best oral and maxillofacial surgical care available, including making every effort to make the payment process simple and convenient for our patients.
We accept a wide variety of insurance plans and payment options which include Visa, MasterCard, Discover, American Express, debit cards, personal checks, cash, Lending Club and Care Credit.
As a service to our patients, we will file claims to the insurance on your behalf for the date of service and, upon patient request, file predeterminations. Please note that any insurance estimate given is not a guarantee of coverage and, regardless of insurance coverage, any unpaid balance is the responsibility of the patient. If you have any questions regarding your insurance coverage or exclusions, we recommend that you contact your insurance company directly.
Please make sure to bring your insurance information with you at your first visit so we can expedite reimbursement.

Please note Fairfax Oral and Maxillofacial Surgery does not accept Medicaid, Medicare, Medicare Supplements, or Tricare Medical.
To put simply, this means we have a contract with the insurance company and are able to offer services for a discounted rate which the insurance company may cover partially. However, we are happy to accept out of network PPO plans as well. These out of network plans may have coverage for out of network providers but without the additional contracted discount and will reimburse at a different benefit amount.
Insurance Notes
- We encourage the discussion of services and their associated fees prior to any treatment rendered in order to avoid costly misunderstandings.
- Your insurance contract is an agreement between you and your insurance carrier. We strongly encourage patients to study the provisions of their insurance prior to surgery as there is significant variation in coverage between companies and plans. We are pleased to seek pre-treatment authorization on request following examination and consultation.
- As a service to you, we will file claims with your insurance carrier. We require a deposit, as well as deductibles and non-covered services to be paid on the day of service. For your convenience we accept cash, checks, and the following credit cards: Visa, Mastercard, Discover, and American Express.
- Balances remaining following insurance reimbursement are your responsibility. Feel free to contact our Billing Department if you have any questions prior to your treatment.
Financing
We are committed to helping you through every step of the treatment process and that includes providing guidance on how you can obtain financing to fund part or all of your out-of-pocket expenses. Our team will discuss these options in-depth during your consultation and many times able to determine financing options on the spot.
