Patient Resource Area
Staff members are available to make appointments and answer questions during our regular business hours from 8:30am to 5:00pm, Monday through Friday. Please notify our staff if you are experiencing pain or an emergency so that every attempt can be made to see you as soon as possible.
We wish to provide the highest level of patient care and meet the needs of all our patients. Occasionally, unexpected events and emergency procedures may cause a delay in your scheduled appointment. We will do our best to minimize any delays. We ask for your understanding so that every patient receives exceptional care. Patients under anesthesia are our top priority!
You may call any of our offices or fill out a appointment request to schedule an appointment. Our scheduling software enables our staff to schedule patient appointments at all of our office locations.

Register Online
First-time patients can save time and fill out the new patient pre-registration forms online before your visit the office. This is an optional step. You may also fill out the forms at our office during your initial visit. Please visit the link below for more info:
Insurance & Financing
Your procedure may be covered by your medical insurance. Please call your insurance provider for more information. Learn more about the insurances we accept and other options for financing your procedure.
Your First Visit
At your first appointment, we will conduct a comprehensive consultation. The doctor will review your dental and medical history, perform an oral exam, and discuss your diagnosis and proposed treatment plans.
HIPAA & Patient Privacy
We take patient confidentiality very seriously and all offices are compliant with HIPAA privacy laws. Please call our office if you’d like to request a records release form. We will also provide the correct form to request your medical records, if necessary.
